Description
IMPACT is a REACH U.S. Center of Excellence for the Elimination of Disparities funded by the Centers for Disease Control and Prevention (CDC). IMPACT is led by community residents, health care providers and researchers, representatives from community-based organizations, faith-based institutions, housing and social service agencies, and local policymakers who have united to understand the needs of the community to help people better prevent and control diabetes by enacting systems, environmental and policy level change.
