For Our New Faculty

Incoming Faculty must complete a manual Financial Interest and Outside Activity Disclosure Statement [PDF] (Preliminary Disclosure Form) within one month of returning a signed Offer Letter. Within 45 days of date-of-hire, you must complete a Report of Relationships with Outside Entities disclosure form online through Sinai Central. You must update your form during the year as you enter into new arrangements or existing arrangements change. All faculty performing research must also complete and submit a separate Financial Interest in Research Disclosure Form with each research application filed with the Grants and Contracts Office (GCO).
