About the Department of Family Medicine and Community Health

The Department of Family Medicine and Community Health, a collaboration between the Icahn School of Medicine at Mount Sinai and the Institute for Family Health, opened on July 1, 2012. The Department delivers a high standard of family health care to the Harlem community and conducts cutting-edge research and education to eliminate health disparities among the underserved. Educational Mission

The educational mission of the Department is to ensure outstanding primary care educational experiences for students, residents, and other trainees in both outpatient and inpatient settings, using a family practice model.

Research Mission

The research mission of the Department is to develop evidence that improves practices and outcomes, addresses barriers to health equity, enhances patient engagement, and formulates health policy.  To achieve this mission, the Department will:  

  • Conduct clinical research directed at diseases and conditions prevalent in underserved populations
  • Engage in health services research focused on primary care and mental health care

Clinical Mission

The clinical mission of the Department is to deliver excellent, accessible clinical care, particularly for patients from medically underserved communities such as East Harlem.  To support its clinical mission, the Department will:

  • Work to refine and perfect models for delivering primary health care, behavioral health care, and dental care for persons of all ages
  • Use  health information technology to enhance the quality and efficiency of care, coordinate care with other MSSM providers, offer  patients access to their health records and interact with the public health care system – all while maintaining the highest levels of privacy and security of personal health information
  • Implement community-based health promotion and outreach programs

Foundational to its overall mission is the Department’s commitment to:  

  • Promote work force diversity
  • Provide a supportive work environment that encourages personal and professional development of Department faculty and staff
  • Care for those experience difficulty accessing health care due to socioeconomic factors, disability, or other factors
  • Work collaboratively with the Family Health Center of Harlem and the Institute for Family Health to enhance all components of the departmental mission
