If your department is listed below, you have an exempt determiner appointed within your department to review your application. Please submit your Exemption Application Form directly to your department's representative.
If your department is not listed below then please submit your Exemption Application Form to email@example.com and the PPHS will review your request.

- Medical Education
- Oncological Sciences
- Preventative Medicine
My department wants to have an Exempt Determiner:
The PPHS has a training program for Exempt Determiners. The training program is the first certification course offered by the PPHS office and involves an in-person training session, completion of a quiz, and mentored review of three exempt projects. If you are interested in becoming an Exempt Determiner for your department, please contact firstname.lastname@example.org.
Please note: PIs cannot make their own exempt determinations. PIs can make their own determination that a project does not involve human subjects research.
