  • Chief of Service Statement Instructions
    The nominating Chief of Service at the affiliate hospital must prepare and sign a statement recommending each candidate for appointment or promotion. The statement should not exceed two pages. The statement must specify candidate's proposed rank and department, and must take into consideration the qualifications for appointment at that rank.

Clinical Associate Professor / Research Associate Professor

  • Clinical Activities: Describe accomplishments to date, demonstrating candidate's emerging senior status in the field. Provide evidence of regional recognition as excellent caregivers.

  • Teaching: Highlight major teaching responsibilities and accomplishments e.g., record of excellent student evaluations.

  • Basic Science and/or Clinical Research: If candidate is involved in research, briefly describe the significance of the research, e.g., extent of impact on the particular field and the candidate's role.

  • Scholarship: Discuss key journal publications, book chapters, presentations, etc. and their impact on the field. If there are key publications where candidate is not listed as first or last (senior) author, it is important to point out candidate's particular contributions.

  • Administration/Service: Describe candidate's administrative contributions at the department and institutional level.

Clinical Professor / Research Professor

  • Clinical Activities: Present evidence of candidate's superior clinical competence, including evidence of regional and national recognition as an excellent caregiver.

  • Teaching: Highlight key evidence of candidate's teaching e.g., record of excellent student evaluations, invitations to teach in Continuing Medical Education Symposia, etc.

  • Basic Science and/or Clinical Research: Briefly describe the significance of the research, e.g., impact on the particular field.

  • Scholarship: Discuss key journal articles, book chapters, etc. and their impact on the field. Highlight innovations in the areas of teaching and/or research. If there are key publications where candidate is not listed as senior author, it is important to point out candidate's particular contributions.

  • Administration/Service: Describe administrative leadership roles, and highlight positive changes that have resulted from the candidate's efforts.

    Submit for new appointments only. Candidates must print their names and sign and date both the top and bottom of the form.

    Note: If candidate is not a U.S. citizen, he/she should only sign the top signature line on form.

  • Reviewer Nominee List (instructions)
    Letters of recommendation will be solicited by Mount Sinai for all senior candidates. Please provide a list of reviewers, using the following guidelines:

    1. 1. Reviewers should be recognized experts in the candidate's field or should be sufficiently familiar with the field to render an informed opinion

    2. 2. For each reviewer, please provide name, title, institution, phone number and if possible e-mail address.

    3. 3. Please do NOT include reviewers who are: members of candidate's own department; individuals with whom the candidate has collaborated (i.e., shared funding, etc.) or published with during the past 3 years.

    4. 4. No more than 2 reviewer selections from the same institution.

  • Clinical Associate Professor/Research Associate Professor – Please provide a minimum of 8 reviewer nominees. Two reviewers may be from Mount Sinai or its affiliates. The list should include regional as well as local selections . Reviewers should hold the rank of Associate Professor, Professor or an equivalent stature.

  • Clinical Professor / Research Professor – Please provide a minimum of 10 reviewer nominees. Two reviewers may be from Mount Sinai or its affiliates. The list should encompass broad regional selections as well as local. Reviewers should hold the Professorial rank or an equivalent stature.

  • Reprints/Supporting Scholarship Material

    • Clinical Professor /Research Professor – submit 3 reprints, or other representations of scholarly work (submission should not exceed 3).

    • Clinical Associate Professor/ Research Associate Professor – this documentation not required.
