Reporting/Disclosure Forms:

  • Report of Relationships with Outside Entities (Annual Form) is completed annually online through Sinai Central. Your form must be updated during the year if you enter into new arrangements or your existing arrangements change.

    • All faculty identified as Investigators on grants submitted to the GCO

What needs to be reported/disclosed:

    • The information from your annual Report of Relationships with Outside Entities will import into this form. All relationships with outside entities that are related to the research must be disclosed

Review Procedure/Management of Financial Interests:

  • Business: Financial interests that present a potential business conflict of interest will be reviewed by the Staff Conflicts of Interest Review Committee (SCCOM).
    Recommended steps to manage or eliminate a conflict are forwarded to the Chief Executive Officer whose decisions are final. For more detail, refer to the Policy on Business Conflicts of Interest [PDF].
  • Research: All financial interests related to research are reviewed by the Financial Conflicts of Interest in Research Committee (FCOIRC). If the FCOIRC identifies a conflict that can be neutralized with a management plan, the research may proceed at Mount Sinai. For more detail, refer to the Policy on Financial Conflicts of Interest in Research.
