For Our Staff

Staff must complete a Report of Relationships with Outside Entities disclosure form annually online through Sinai Central if they:

• Perform research at Mount Sinai, or
• Are a Department or Division Administrator, or
• Have a potential conflict of interest that requires institutional reporting.

You must update your form during the year if you enter into new arrangements or existing arrangements change. If you are performing Human Subjects research, you must also complete and submit a separate Financial Interest in Research Disclosure Form with each research application filed with the Grants and Contracts Office (GCO).

If you are unsure if you need to complete the disclosure forms, please contact us directly or you may contact the Program for the Protection of Human Subjects (PPHS) if you are involved in Human Subjects research.
