Preface
This collection was transferred from the Department of Community Medicine to the Archives in a series of accretions during 1990-1994 and again in 2000. Files were also donated to the Archives by Dr. Deuschle's widow, Mrs. Jeanne Deuschle. Four boxes of material relating to Dr. Deuschle's work with the Navajo nation were given to the tribe by Mrs. Deuschle in 2005, including slides, photographs, films, books and files from his years of service in Arizona.
The Correspondence Series was processed by Maria LaCaille in 2004. She also wrote the narrative portion of the finding aid. Elizabeth DuRocher processed the Alpha II and SERP series in 2009. Barbara Niss processed the remaining pieces, updating and finalizing the finding aid in 2012.
Biography
Considered by many to be the "Father of Community Medicine," Kurt W. Deuschle served as Chairman of Mount Sinai's Community Medicine Department from 1968 until 1990, when Philip Landrigan M.D. took over as Chairman. Deuschle remained on faculty as a Distinguished Service Professor of Community Medicine until his death in 2003.
Deuschle was born in 1923, and his family emigrated from Germany to the United States the following year. They settled in Baden, Pennsylvania, where Deuschle spent his childhood. At the age of eighteen, Deuschle went to Kent State University, graduating cum laude in 1944. That same year he began medical school at the University of Michigan and graduated four years later.
Deuschle's first professional experience after his residency at SUNY Syracuse proved pivotal. From 1952-1954, he worked in the United States Public Health Service as Chief of the tuberculosis program at the Navajo Medical Center in Fort Defiance, Arizona. During this time he developed his concept of community health programs and was involved in the first field trials of the tuberculosis drug Isoniazid. Drawing from this experience he co-authored The People's Health: Anthropology and Medicine in a Navajo Community with medical anthropologist, John Adair. Published in 1970, the book was considered a major contribution to medical anthropology and Navajo health. Following his time in the U.S. Public Health Service, Deuschle went on to be the director of the Navajo-Cornell Field Health Research Project, where he worked with Walsh McDermott M.D. and John Adair.
In 1960 Deuschle created the first U.S. Department of Community Medicine at the University of Kentucky School of Medicine. Under Deuschle's leadership, the Department was committed to improving rural health. Known as the "Kentucky Model," the University of Kentucky's Community Medicine Department was much admired by likeminded medical professionals and inspired other universities to develop their own community medicine programs.
In 1968, with the opening of the Medical School imminent, the first Chairman of the Department of Community Medicine at Mount Sinai, George W. James M.D., who was also Dean of the School, recruited Deuschle to take his place as Chairman. James had only intended to serve as Chairman long enough to get the Department running. And so, in a move that would take him from the health problems of rural Appalachians to those of inner city residents, Deuschle was named Chair of the Mount Sinai Department of Community Medicine in 1968. In 1973, his position was endowed and his title became the Ethel H. Wise Professor and Chairman of the Department of Community Medicine.
As the first full-time Chairman, Deuschle played a large role in shaping the research focus and mission of the young department. Many of these interests, not surprisingly, revolved around the local community of East Harlem. Additionally, Deuschle's interest in international medicine also influenced the work of the Department. Once the Department was settled, Deuschle established a Division of International Medicine, led by Samuel Bosch, MD, that worked with local physicians to create or improve health programs in several countries including Nigeria, China, Vietnam, Turkey, Columbia, Jamaica, Mexico, the Dominican Republic, and the People's Republic of China.
During his career, Deuschle was actively involved with a large number of professional associations. He served on the World Health Organization's Expert Advisory Panel on Professional and Technical Education of Medical and Auxiliary Personnel from 1972-1977, and was an early member of the Institute of Medicine of the National Academy of Sciences, elected in 1971, the second year of the program. Deuschle was also involved in local health issues, serving on the New York City Board of Health from 1982-1989. Taking his leadership position at Mount Sinai very seriously, Deuschle sat on 28 internal boards and committees during his career at the school, including the Presidents Advisory Group, the Executive Faculty Board, the Affirmative Action Committee, and the Ethics Committee.
In honor of his dedicated service to the profession, Deuschle received several awards and prizes. The 1975 Award for Excellence in Domestic Health from the American Public Health Association citation includes these lines: "Perhaps because he seems to see opportunity in each situation and potential in every person, he continues to lead at the cutting edge of major innovations, yet retains the respect and esteem of his co-workers. We look with wonder and admiration upon this uncommon man and…his singular contributions to the advance of American public health." Later awards include the Mayor's Award of Honor for Science and Technology in 1989 and the Duncan Clark Award from the Association of Teachers of Preventive Medicine in 1990. Mount Sinai also honored Deuschle with the Jacobi Medallion from the Mount Sinai Alumni Association in 1989 and the Alexander Richman Commemorative Award for Humanism in Medicine in 1991.
Deuschle published over 100 journal articles during his career. Generally, these works centered on issues such as Navajo health, community medicine education, health manpower, health care delivery, and international health. He also published fourteen books and monographs including The People's Health (1970) about the Navajo-Cornell Health Project, Health Manpower Planning in Turkey: An International Research Case Study published by Johns Hopkins Press in 1968. Drawing from his experience studying health care delivery in Harlem, in 1972 Deuschle also contributed a chapter in an edited volume entitled A Health Care Plan for East Harlem - Now.
Upon his retirement in 1990, two graduates wrote appreciatively of Deuschle's leadership:
Kurt's grace and wisdom, his insight, kindness, and humor, his loyalty to and his intense concern for and understanding of his students and colleagues are, if not unique, at least rare features for a professor, or even a civil servant…He has enabled his students to see their patients in the context of a community. 1
Following a battle with Parkinson's disease, Deuschle passed away on February 10, 2003.

Departmental History
The Department of Community Medicine During the Early Years (1966-1990)
The creation in 1966 of the Department of Community Medicine (DCM) in Mount Sinai School of Medicine exemplified the young school's commitment to the health needs of the community. The Department was, in fact, designated in the Medical School's charter as a "third faculty", coequal in status with the basic science and clinical faculties. Mount Sinai's Department of Community Medicine was the first program of its kind in an urban environment. For the first three years of its existence, George James, MD, former New York City Health Commissioner and the new Dean of the School of Medicine, served as Chairman and Director of the Department. The 1966 Annual Report of The Mount Sinai Hospitalstates that: "The Department of Community Medicine is responsible for those Hospital activities which relate to the prevention of disease and the care of ambulatory patients." (p.16) There were initially three divisions: Environmental Medicine, led by Irving J. Selikoff, MD; the Division of Medical Practice (established July 1966), which was responsible for supervising the emergency room and the ambulatory care clinics and was led by Charles A. Goodrich, MD; and the Division of Nutrition (established December 1966), which "investigated the role that foods play in the development of disease and the maintenance of health." (p.17) This was led by George Christakis, MD and was a joint program with the Department of Medicine. The first research efforts of the department involved creating a database of information that would tell Mount Sinai who exactly their patients were and why they had come to the Hospital.
In 1968 Dr. James recruited Kurt W. Deuschle to become the Chairman of Community Medicine and the scope of the department began to change. The mission of the Department of Community Medicine remained the same: practitioners examined the social and environmental causes of disease in population groups and strove to understand individuals as members of populations. But, under the Deuschle model, Mount Sinai could not solve all the local health problems alone, but would instead take on the role of facilitator. He believed that direct community involvement had to play a fundamental role in improving health conditions. He envisioned that, "The program will provide the academic leadership to identify and work towards solutions of community health problems." (1969 Annual Report, p. 111.) This meant that the Department would assess the needs of the community and then work with the community to match these needs with the appropriate resources. These early efforts to build the Department were supported by the Commonwealth Fund.
The Mount Sinai Annual Reports for 1968 and 1969 describe the changes that occurred in the department over Dr. Deuschle's first year. Whether for philosophical reasons or more practical concerns, having the Outpatient Department and Emergency Room within Community Medicine under the management of a Division of Medical Practice, renamed the Division of Health Care under Dr. Deuschle, did not last. It is not clear from these records exactly why this did not work. The DCM did continue work in the clinics and provided statistics and research data to study the programs, but direct supervision of the clinical components ended in 1969/70.
The report covering 1968 lists these areas as divisions of the Department: Educational Coordination, Division of Health Economics, Division of Nutrition, Division of Social Work, Division of Health Care, Division of Behavioral Sciences, and the Division of Environmental Medicine. By 1969 these divisions had been joined by a Division of Nursing Research led by Helen Simon, Ph.D. and a Division of International Health, led by Rodolphe L. Coigney, MD.
With the arrival of Dr. Deuschle, research assumed a central role in the department, in particular looking at current conditions in the surrounding community of East Harlem. In one of the first research projects begun after Deuschle came as Chairman, Louise Johnson surveyed the health concerns of local residents and published them in the report, The People of East Harlem. This was then followed in 1979 by a Report on Primary Care in East Harlem, which was later used as a guide for local health planning. The Coordinator of Community Relations, a position created by Deuschle and initially held by social worker Max Bowens, took on the role of facilitator and strove to mediate relations between East Harlem residents and Mount Sinai in an effort to make Mount Sinai more responsive to the needs of the community. With time, this role expanded and grew into the independent Community Relations Department.
The Department of Community Medicine encountered several challenges in its early years and funding for projects was always an issue. The difficult financial times experienced during the 1970s resulted in cutbacks in the Community Medicine program and curriculum. As time went on, divisions that had formed under strong leadership closed with the departure of the director and the lack of continuing funds. The Division of Nutrition closed when the chief, George Christakis, M.D., left in the 1970s. The Division of Health Economics, led by Victor Fuchs, Ph.D. from 1968-74, was eventually folded into another unit. (As areas of research overlapped, new groupings were formed and a conscious effort to streamline the Department was enacted once Dr. Landrigan took over the Chairmanship.
Undergraduate Training
In addition to his innovative ideas on medical research and intervention, Dr. Deuschle also had a special approach to medical education and training. He created an Education Division to coordinate the curriculum and teaching efforts of the Department of Community Medicine. This was ably led by Bessie S. Dana, M.S.S.A. for many years and then by Alan Silver, MD. Dana was also active with Arthur H. Aufses, Jr., MD in the creation of the School of Medicine's first year multidisciplinary course called Introduction to Medicine, in which the Department faculty taught.
Dr. Deuschle's education plans focused on instilling undergraduate medical students with a solid understanding of methodological research skills and clinical epidemiology. Initially a first year course was offered as well as a fourth-year clerkship, an important component of the Department's contribution to the Mount Sinai approach to medical education. In this clerkship, students conducted a population-based research project with the hope that as a result of this experience, students would gain a basic understanding of community medicine practice and population-based research methodologies. This clerkship eventually became required for all third year medical students. Today it is part of the Ambulatory Care clerkship.
Graduate Training
The Community Medicine Department began offering residency programs in General Preventive Medicine in 1971 and in Environmental Medicine in 1976. These continue today. Fellowships in the Department were created in 1966. In 1988 a Master of Science degree in Community Medicine was created to formalize the educational offerings and the residency programs became part of the Masters program. In 2003 the New York State Education Department approved a change in the award from that of a Master of Science degree to a Master of Public Health (MPH) degree. The change was effective immediately, so students graduating from the program that spring were eligible to receive the MPH degree.
Divisions of the Department, 1968-1990
Information on the Medical Practice Division, the Education Division, the Nutrition Division and the Health Economics Division is found above. Nothing is known about the Nursing Research Division created in 1969.
International Division
An early version of this division was established in 1969 but it did not last as a formal division. In 1979, the International Division of Community Medicine was again created, this time with encouragement from Dean Thomas Chalmers. This Division sought to "promote commu5nity development through improvement in health and health care delivery systems around the world."(2) The first project, conducted in La Romana in the eastern Dominican Republic, assisted in the organization of a comprehensive health system in the region. Following this, in 1983, the Department created the Charles G. Bluhdorn Professorship in International Community Medicine, which was the first of its kind in the United States. This was funded by the Gulf + Western Corporation. Samuel Bosch, M.D. was appointed the first Bluhdorn Professor and Chair of the Division and was head of the International Division until his retirement in 1990. Under Bosch, the Division had three primary goals. First, the Division strove to enhance health planning and education activities that promoted equitable distribution of resources. Second, the Division's mission was to foster professional development in population-based health planning, primary care organization, health promotion and health services administration. Finally, the Division sought to foster community-oriented health services research internationally. Funded by the Fogarty Foundation, the Department began offering international fellowships in occupational and environmental medicine in 1996.

Division of Behavioral Sciences
When the Department of Community Medicine first opened in 1968, there was a separate Division of Behavioral Sciences that was headed by Samuel W. Bloom, Ph.D., a well known medical sociologist. Dr. Bloom held a CUNY-funded chair and was very active with the Graduate Center of The City University of New York. Within this Division there was also an Office of Applied Social Research, headed until 1989 by Jane Moss, Ph.D., another sociologist. This group created studies on various aspects of The Mount Sinai Hospital as a care provider, with particular emphasis on studying the structure and provision of ambulatory care by the Hospital. The Department of Community Medicine was actively involved in Mount Sinai's efforts in the 1970s to reshape its medical clinic, which led to the creation of the Internal Medicine Associates in 1982. This office also studied the demographic data of the Mount Sinai patient population, and created reports at the request of the Hospital administration to help guide planning efforts.
The Mount Sinai-Irving J. Selikoff Clinical Center for Occupational and Environmental Medicine
The Environmental Science Laboratory was created in 1963 and moved to the Department of Community Medicine in 1966 and would form the base for the Division of Occupational and Environmental Medicine. Irving Selikoff, M.D served as head of the Laboratory and then the Division from its inception until 1985. During this time the Division conducted a variety of research studies aimed primarily at occupational medicine and is best known for its pioneering clinical and epidemiologic studies of the health effects of exposures to asbestos, lead and PCBs. In 1973 the Division was revamped and re-named the Environmental Health Sciences Center as a result of additional funding provided by the National Institute for Environmental Health Sciences. Around 1979, with funding from the National Institute for Occupational Safety & Health (NIOSH), Dr. Selikoff formed the New York-New Jersey Education and Research Center in Occupational Health & Safety, a multi-institutional education and research consortium in occupational safety and health that continues at Mount Sinai to the present and supports the residency program in occupational and environmental medicine.
In 1985, Deuschle and Selikoff recruited Philip Landrigan, MD to take over as Director of the Division. The Mount Sinai-Irving J. Selikoff Center was dedicated in December 1987, and by 2011 had grown into the largest academic provider of clinical services in occupational medicine in the New York City area. The Center also housed the Irving J. Selikoff Asbestos Archives and Research Center, which contained some of Selikoff's research data and papers. (These have since been transferred to the Mount Sinai Archives and are closed.) Landrigan served as Director of the Division until taking over for Deuschle as Chairman of the Community Medicine Department in 1990. Under Landrigan, who was trained in pediatrics, the Division concentrated on studying the links between environmental exposure to toxins and ill health among children.
Division of Environmental Health Sciences
This research Division was founded under the leadership of Mary S. Wolff, PhD., and is dedicated to "understanding, elucidating and preventing" diseases caused by environmental exposures. (3)

Division of Social Work
The Social Work Department had a long history at Mount Sinai prior to the opening of Mount Sinai School of Medicine. Originally founded in 1907 by Sigismund S. Goldwater, M.D., Mount Sinai's Social Welfare Department was the first of its kind in New York City. In 1915, the Department was renamed the Social Services Department. Doris Siegel ran the division from 1954 until her death in 1971. Upon the opening of the school in 1968, a Division of Social Work was established within the Department of Community Medicine. In 1969 the Edith J. Baerwald Professor of Community Medicine endowed chairmanship was established, the first such chair in a U.S. medical school and the first endowed chair at Mount Sinai to be held by a woman, Doris Siegel. Siegel's successor, Helen Rehr, D.S.W, was a leader in her field and is most well known for her work on the aging population, and the application of public health principles to the field of social work. Gary Rosenberg, Ph.D., currently holds the Baerwald Chair. The Division serves the patient needs of The Mount Sinai Hospital, participates in the educational efforts of the Department of Community Medicine, and provides social work education to the broader community.
Scope and Content Note
This collection is broken down into several series, listed below. There are two General Alphabetical Series due to the collection being received in pieces and processed over a period of time. Some folders and one additional box were later added to the original Alphabetical Series so like material would be found in one place. These are generally labeled with the letter 'a' after the folder number so as not to disturb the original designation.
One strength of this collection is that it spans Dr. Deuschle's entire career, with some documentation of his Kentucky years, as well as his travels abroad to study and advise on community needs. Naturally, there is also extensive documentation on the Mount Sinai Department of Community Medicine (DCM) but it is spread out into many files and more difficult to piece together. The newsletters and annual reports provide wonderful insights into the goals and efforts of the Department over time. The best way to track the work of particular divisions or areas is to search under the names of the individuals involved. Also, please note that the Archives has a separate series of records for the Division of Occupational and Environmental Medicine.
There were some photographs included with the collection that have been filed in the Archives' photograph collection under Deuschle. The slides that were received are with the collection and form a separate series.

Correspondence Series, 1960-1996: Box 1-11
This series is arranged alphabetical by correspondent, and thereafter chronologically. The majority of correspondents were from outside of Mount Sinai. A significant number of reports and articles that were sent to Deuschle for comment are also contained in this collection. Most letters in this series were written in the 1970s and 1980s and many were from long-time correspondents of Deuschle's.
In the 1970s social medicine gained increasing popularity among students, medical practitioners and academics. Deuschle's role as a leader in this developing profession is documented here, as are the relations between Mount Sinai's Department of Community Medicine and related departments at medical schools across the United States. Those seeking to employ the community medicine approach in their department, or those who wanted to start similar academic programs, often wrote asking for Deuschle's guidance. How to incorporate community medicine into a traditional medical education was a frequent topic. Additionally, the numerous C.V.s contained in this series illustrate the career paths of those interested in community medicine.
Deuschle's early interest in Native American health is documented in several of the letters. Although those interested in his work with the Navajo would find few explicit references to this phase of his life, there are sporadic references to this work in the correspondence, collected clippings and journal articles.
The health status of New York City, particularly in East Harlem, is another frequent topic in this series. Many New York City public health professionals and clinical advocates would send Deuschle reports on their various projects for his comments. These are a valuable source of documentation on both New York City health problems in the 1970s and 1980s and the many interventions developed to address them.
Although overall this series concerns Deuschle's collaborations outside of Mount Sinai, Deuschle makes frequent comments regarding the Department's tenuous financial situation. These frequent grumblings are testimony to the financial troubles of the 1970s, which had quite an impact on the Department.
These files are particularly strong in documenting Deuschle's international interest. Turkey features prominently in this series, as some of the most detailed and passionate letters were from Turkish physicians struggling to apply community medicine principles in what they described as an unsupportive environment. Deuschle's replies to these impassioned letters illustrate his role as empathetic mentor and confidant, as he invariably responded with patient words of encouragement.
General Alphabetical Series I, 1956-2000: Box 12-17
These subject files reflect the workings of the Office of the Chairman and provide much information on the department. Of special note are the departmental annual reports and the departmental reviews that were conducted in the 1980s. This series also has files on the relationship between this group and the East Harlem community, including the early studies by the department in the community. There are also files on the Community Medicine teaching program, the department's dealings with affiliated institutions, and reports on the Chairman's activities to the Lavanburg-Corner House, sponsors of the Ethel Wise Chair of Community Medicine. Deuschle's continuing interest in American Indians, and specifically the Navajo, is evidenced by the files relating to his two editions of The People's Health, as well as files on the Indian Health Service and the Navajo.
Reprint Series, 1953-1995: Box 18
These are the articles authored by Kurt Deuschle over his career. They are arranged chronologically. A box of reprints and photocopies of articles by others has been discarded.
Subcommittee on Ethical Research Practices Series (SERP), 1979-1988: Box 19-20
Dr. Deuschle was Chairman of the Subcommittee on Ethical Research Practices of the Research Administrative Committee (RAC) from its inception in 1983 until 1988. A major task of this group was initially to write institutional guidelines for misconduct in research. Comments were solicited from other institutions and many drafts were created. Those appear in Box 19. SERP also served as a general Ethics Committee and investigated instances of alleged fraud and misconduct. These files (Box 20) are closed to research.
Personal Series, 1966-1995: Box 21-22
This series contains material not related to the Dept. of Community Medicine but specifically to Dr. Deuschle. There is an exchange of letters between Deuschle and George James, MD about coming to Mount Sinai School of Medicine. There are also a few folders on Dr. Deuschle's proposed and completed sabbatical leaves. The manuscript papers found here do not seem to have been published and remain here as opposed to the Reprints file.
The bulk of this series is clustered around Dr. Deuschle's later years at Mount Sinai. There are many letters of support and congratulation as he celebrated various events such as Dr. Kurt Deuschle Day at Mount Sinai in 1990; winning several awards; having a Festschrift in 1992; and reaching Emeritus status. Official material on the transition to a new Chairman is found in the Alpha Series under "Search Committee".
International Series, 1951-1989: Boxes 23-26
This series contains information about trips taken by Dr. Deuschle over the years as a consultant or as part of a study group. The bulk of the material relates to two trips to Turkey in 1963 and 1984. The initial trip was to do a medical manpower study under the aegis of Johns Hopkins University. Earlier material represents articles on the countries visited. Other destinations contained here are: China, La Romana (Dominican Republic), Nigeria (Lagos), Saudi Arabia, Vietnam and the West Indies. There are also several files here on international visitors that came to the Dept. of Community Medicine. These are organized by country. There is also a file on visitors from around the U.S.
General Alphabetical Series II, 1954-1995: Box 27-32
This contains correspondence and information primarily by and about outside professional groups and agencies. This includes organizations such as the American College of Preventive Medicine, the Association on American Indian Affairs, Inc., the World Health Organization and U.S. government agencies as well as programs such as the Health of the Public and the Infectious Disease Research Institute. The latter supported the research of Gladys Hobby, part of the Cornell medical group that Dr. Deuschle remained close to throughout his career, along with Walsh McDermott. There are also some files relating to the Community Medicine department and its operation.
Foundations Series, 1968-1995: Box 32-34
This series is of interest because it shows what projects the Department of Community Medicine wanted to pursue. Of particular interest here are the Commonwealth Foundation files as that group was an early supporter of the Department and Mount Sinai School of Medicine itself. This includes a great deal of information about the early years of the Department.
Faculty Series, 1948-1993: Box 35-39
This series contains two kinds of files, both organized by faculty members' names. Some individuals only have appointment information, but Dr. Deuschle's office also used these files to store substantive correspondence on professional and personal issues. This series contains some of the only documentation in the collection on the Health Economics Division as well as the Division of Occupational and Environmental Medicine. The non-appointment material has been separated from the appointment data, which has restrictions due to privacy issues. These files may be used, but only after review by the Archives staff to remove personal information such as Social Security numbers or upon proof of the death of the faculty member involved.
Recommendations Series, 1968-1994: Box 39-41
These files contain letters on behalf of Mount Sinai medical students, faculty and staff members of the Department of Community Medicine. There are also letters on behalf of others outside the Department who were friends and/or colleagues of Dr. Deuschle. In the case of the medical student references (usually for residency positions), where identical letters are sent to multiple residency programs, only a sample letter was retained. Please note that this series also contains letters written for department members who also have files in the Faculty Series.
Slides Series, 1952-1981: Box 42
These are 35 mm. slides (mostly color) taken on trips. There are also some data slides used in lectures. The largest group of slides deals with East Harlem and provides a fascinating glimpse of what the community looked like when Dr. Deuschle came to Mount Sinai.
