Program Purpose

This PGY2 ambulatory care pharmacy residency program builds on Doctor of Pharmacy education and PGY1 pharmacy residency program training to contribute to the development of the clinical pharmacist in specialized areas of ambulatory care practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care and other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification in ambulatory care.

Program Overview

We have designed our PGY2 Ambulatory Care Pharmacy Residency program to foster our residents' ability to apply evidence-based medicine within various population health models or other advancing ambulatory care pharmacy settings. We use interdisciplinary and independent learning opportunities in a variety of clinical areas to achieve this. Strengths of this program include longitudinal patient-centered care within interdisciplinary teams as well as numerous opportunities in population health management and research. We focus on refining skills to manage chronic disease states, transitions of care, medication reconciliation, and care management. We promote wellness and utilization of preventative services. 

You will work longitudinally with patients, which will give you ample opportunity to build relationships and follow patients over time. You will be able to rotate within MSHP’s accountable care organization (ACO) and participate in drug information and quality improvement initiatives. Additionally, you will have opportunities to provide patient care in other therapeutic areas such as cardiology, endocrinology, gastroenterology, nephrology, and pulmonology.

Population Health at a Glance

As a population health team, we contractually manage a set of patients longitudinally across all care settings. One of our responsibilities is to manage unnecessary and avoidable health care costs while focusing on preventive care and disease management. Pharmacists are uniquely able to offer clinical services strongly aligned with the shifts to value-based care. You will also gain the opportunity to support patients across traditional episodic care management, their managed care insurance providers and local community-based organizations.

Description of the Practice Site

The Mount Sinai Health System includes The Icahn School of Medicine at Mount Sinai, seven hospital campuses in the New York metropolitan area, and a large, regional ambulatory footprint. We are internationally acclaimed for excellence in research, patient care, and education. Mount Sinai Health Partners’ clinically integrated network embraces population health management through a commitment to care management and new payment models that reward value-based care and greater accountability for cost and quality. As pharmacists are critical in providing quality and effective care, you will be providing direct patient care and research across practice sites that will support our value-based care initiatives. You will work in an environment that recognizes the value of credentialing pharmacists, and fosters advanced pharmacy practice through collaborative drug therapy management agreements.
