Curriculum

The Cardiovascular Fellowship at The Mount Sinai Hospital curriculum offers an unparalleled educational experience that combines hands-on clinical training with advanced research opportunities. Fellows work directly with world-renowned experts, including Deepak L. Bhatt, MD, MPH, MBA, and Valentin Fuster, MD, PhD, to gain exposure to emerging therapeutics and innovative treatment approaches. The structured didactic program includes regular multi-modality conferences, where leading experts discuss the integration of state-of-the-art cardiac imaging and diagnostics, visiting professor lectures from international authorities, and engaging panel discussions on divisive topics in cardiology.

Our first-year clinical rotations include:

Cardiac Catheterization
Fellows develop proficiency in venous and arterial access, interpretation of coronary angiograms and hemodynamics, assessment and management of acute coronary syndromes, and mechanical support devices. Focus is on evaluating patients for catheterization appropriateness and performing supervised diagnostic procedures.

Electrophysiology
Training in this area emphasizes diagnosing and managing complex cardiac arrhythmias through consultative services. Our fellows also explore device interrogation and programming, with hands-on exposure to invasive and noninvasive evaluation techniques.

Echocardiography
After completing a summer boot camp, fellows develop proficiency in technical acquisition and interpretation of transthoracic echocardiograms, working directly with expert sonographers and attending physicians in hands-on image acquisition and interpretation sessions.

Nuclear Cardiology
Fellows master all stress test protocols, understanding their indications and contraindications. They participate in direct study reading with attendings and learn basic nuclear imaging interpretation.

Cardiology Rotation with Dr. Bhatt and Dr. Fuster
Fellows have the unique opportunity to work closely with Dr. Bhatt and Dr. Fuster for 8-10 weeks, managing both inpatient and outpatient populations.

Advanced Cardiac Imaging
Fellows learn the basics of cardiac CT and MRI interpretation, including calcium scoring, coronary computed tomographic angiography, and fundamental cardiovascular magnetic resonance sequences for cardiac function assessment.

Cardiac Critical Care
Fellows lead a team managing complex cardiac conditions, including acute coronary syndromes, advanced heart failure, and mechanical support devices. They perform bedside procedures, including pulmonary artery catheters and transvenous pacemakers.

Heart Failure
Rotations on consultative services introduce our fellows to advanced heart failure management. This includes temporary mechanical support and post-transplant care, preparing them for complex patient scenarios.

Cardiology Consults
Fellows hone their teaching and leadership skills through the consultation service, where they manage a wide range of cardiovascular conditions while supervising house staff and medical students.

Continuity Clinic
Outpatient practice begins for our fellows in the Continuity Clinic. Here, our fellows begin developing their outpatient practice under close supervision, learning to manage common cardiovascular conditions in the ambulatory setting.

Our second-year clinical rotations include:

Cardiac Catheterization
Fellows take on increased responsibilities with greater procedural participation and independence, tailored to their career interests and goals. Emphasis is placed on complex interventions and advancement of technical skills.

Electrophysiology
Fellows gain increased exposure to advanced ablative procedures and complex device management. They participate in innovative device trials and cutting-edge procedures while developing greater autonomy in patient care decisions.

Echocardiography
Fellows advance to performing stress and transesophageal echocardiograms under supervision. Most achieve Level II training and begin participating in structural intervention guidance.

Nuclear Cardiology
Training advances to subjective and objective quantification of anatomic perfusion abnormalities and CT correction. Fellows work toward Level II training certification with increased independence in study interpretation.

Training progresses to more complex imaging interpretation, including CT-FFR, structural heart planning, and advanced cardiovascular magnetic resonance applications for various cardiac conditions.

Cardiac Critical Care
Fellows take on cases that are more complex and supervise first-year fellows, with increased responsibility for mechanical circulatory support device management and critical care decision-making.

Vascular Medicine
Fellows receive comprehensive training in vascular disease management through a collaborative approach with vascular surgery, interventional radiology, peripheral interventional cardiology, rheumatology, neurology, and neuroradiology. Training includes time in outpatient clinics, inpatient consultations, and the vascular diagnostic laboratory, with exposure to endovascular and surgical procedures.

Adult Congenital Heart Disease
Fellows rotate through the adult congenital heart center, participating in weekly clinic sessions evaluating adult congenital heart disease patients at all levels of complexity. Training includes exposure to dedicated multi-modality imaging in coronary heart disease, including echocardiography, cardiovascular magnetic resonance, and computed tomography angiography imaging. Fellows may also participate in the congenital cardiac catheterization suite and specialized multidisciplinary clinics focusing on single ventricle Fontan patients and high-risk pregnancy management.

Heart Failure
Focus shifts to complex decision-making for advanced therapies, including LVAD management and transplant care, with increased autonomy in patient management.

Cardiology Consults
Fellows take on consultations that are more complex and begin developing subspecialty expertise based on their career interests.

Continuity Clinic
Fellows manage a more complex patient panel with greater autonomy, while maintaining continuity of care throughout their training.

The third year of fellowship is designed to be highly customizable, allowing fellows to prepare for their chosen subspecialty while ensuring a solid foundation across all core areas. While certain rotations remain mandatory for all fellows (including two weeks of Cardiothoracic Surgery and two weeks of Senior Cardiac Intensive Care), the majority of the year is tailored to their individual career goals and interests. Additional rotations include:

Cardio-Oncology
The cardio-oncology program provides collaborative care between cardiac and cancer specialists to optimize cancer treatment while minimizing heart-related side effects. Established in 2013, it serves patients with diverse cancers at Mount Sinai's Hess Center, supporting advanced treatments and clinical trials. Fellows participate in multidisciplinary care and research opportunities spanning clinical, imaging, and outcomes studies in this specialized field.

Lipoprotein Disorders
The Cardiometabolic Unit combines patient care with scientific research, focusing on lipoprotein disorders and cardiovascular prevention. The program offers comprehensive management for patients with genetic lipid conditions while conducting clinical trials of new treatments. Daily outpatient services at the Lauder Cardiovascular Center integrate cardiology, diabetology, nutrition, and exercise therapy.

Cardio-Metabolism
The Cardiometabolic Service provides integrated care to reduce cardiovascular disease risks through multidisciplinary collaboration. Located in The Marie-Josée and Henry R. Kravis Center for Cardiovascular Health at Mount Sinai Heart, specialists from cardiology, endocrinology, and internal medicine deliver personalized, evidence-based treatments. The center encompasses clinical care, cardiac rehabilitation, fitness programs, nutrition services, and community education, with seamless connection to inpatient consultation. Fellows learn risk assessment, lifestyle medicine, rehabilitation, nutrition, diabetes and obesity management, and outpatient care coordination.

Third-year fellows can choose from several specialized pathways:

Clinical Imaging Track
For fellows interested in imaging, this track offers two focus areas: general imaging for outpatient practice and advanced echocardiography with specialization in 3D and structural imaging.

Comprehensive training across multiple imaging modalities ensures our fellows gain expertise in their chosen area while maintaining a broad skill set.

Procedural Track
The Procedural Track is designed for fellows planning careers in interventional cardiology or electrophysiology. Fellows spend increased time in the catheterization and EP labs, participating in complex procedures and advanced cases. The training provides a strong foundation for additional subspecialty training while maintaining continued proficiency in general cardiology.

Advanced Clinical Track
Fellows interested in heart failure, cardiac critical care, or adult congenital heart disease focus their training in these specialized areas. These pathways integrate hands-on patient care with opportunities for research and specialized procedures, supplying fellows with the skills necessary for these challenging fields.

All tracks maintain a strong emphasis on research opportunities, recognizing the importance of academic advancement in cardiology. All our fellows are encouraged to participate in ongoing clinical trials, quality improvement initiatives, and independent research projects regardless of their chosen track. The flexible structure of the third year ensures that fellows graduate ready to pursue advanced fellowship training or begin their careers as independent practitioners.
