Prepare for Your Visit

Before Your First Neurological Examination Visit

To  prepare for your first appointment, please take these steps:

  • Please forward copies of your medical records prior to your appointment, including:
    • Medical notes from any previous medical evaluation performed for the condition you are seeing us for and primary provider’s notes for any visit you had during the year prior to your appointment
    • Recent blood and/or cerebrospinal fluid (CSF) test reports
    • Brain imaging (MRI, CT, PET scan): please bring images burned on CDs or actual brain imaging films for review
    • Prior neuropsychological test reports if any
    • Complete list of all current medications (or alternatively bring all medication containers)

  • Please bring completed Neurology New Patient Form
  • Plan to bring a family member or friend who knows you well. Our evaluations would be incomplete without speaking with someone who knows you well. This person can be your spouse, another family member, or a close friend.
  • Please arrive 15 to 30 minutes before your scheduled appointment to complete check-in.

Before Your First Neuropsychological Evaluation Visit

A neuropsychological evaluation is a procedure that requires a referral from a healthcare professional. To be scheduled for a neuropsychological evaluation, we will ask you to forward your doctor’s referral or have your healthcare professional forward the referral to us.

Please forward copies of your medical records prior to your appointment, including:

  • Doctor’s notes from any previous medical evaluation performed for the condition you are seeing us for and/or recent provider’s notes for any visit you had for a year prior to your appointment
  • Brain imaging (MRI, CT, PET scan): please bring images burned on compact discs (CDs) or  actual brain imaging films for review.
  • Prior neuropsychological test reports if any
  • Complete list of all current medications (or alternatively bring all medication containers). This information is part of the Neuropsychology Intake Form.

Plan on bringing a family member or friend who knows you well. Our evaluations would be incomplete without speaking with someone who knows you well. This person can be your spouse, another family member or a close friend.

Preparing for a Neuropsychological Evaluation

There is nothing particular you need to do to prepare for a neuropsychological evaluation, but there are a few recommendations for you to be at your best when you arrive:

  • Get a good night's sleep.
  • Eat an appropriate meal prior to your appointment.
  • Feel free to bring water and/or a snack with you.
  • Take all of your medications as usual unless you are directly instructed to do otherwise.
  • If you use glasses, contact lenses, or hearing aids, make sure you have them with you.
