Neurocritical Care Pre-Residency Fellowship and Critical Care Medicine Pre-Residency Fellowship at Mount Sinai Hospital

These Pre-Residency Fellowships, under the umbrella of the Institute for Critical Care Medicine at Mount Sinai, are each designed as a 12-month program for medical school graduates aspiring to strengthen their clinical foundation before embarking on formal residency training. Applicants to each of these programs will be considered for placement in either program.

  • Neurocritical Care Pre-Residency Fellowship
    This immersive fellowship experience is dedicated to exposing participants to the intricacies of neuro-intensive care, ensuring they are well prepared to handle critically ill neurology and neurosurgery patients. There is also in-depth exposure to internal medicine and critical care medicine.
  • Critical Care Medicine Pre-Residency Fellowship
    This fellowship provides an immersive experience focusing on providing a thorough understanding of internal medicine and critical care medicine.

Meet the Director

Clinical Rotations and Rotational Flexibility: The 12-month duration will be primarily in either Neurosciences Intensive Care Unit (NSICU) at Mount Sinai West; or the Medical Intensive Care Unit (MICU) and Surgical Intensive Care Unit (SICU) at Mount Sinai Queens; or Mount Sinai Brooklyn, with the opportunity to rotate at the other sites

  • Neuro ICU (NSICU)
    This comprehensive experience ensures that participants gain a well-rounded perspective in critical care and internal medicine, enriching their clinical knowledge and procedural skills. Fellows become thoroughly prepared to manage a diverse array of complex neurological and medical emergencies, including ischemic stroke, intraparenchymal hemorrhage, subarachnoid hemorrhage, traumatic brain injury, brain tumors, status epilepticus, and neuromuscular crises such as myasthenic and Guillain–Barré syndromes. The rotation emphasizes hemodynamic management, neuroprognostication, ventilator weaning in neurogenic respiratory failure, and multidisciplinary collaboration with neurosurgery, neurology, and rehabilitation teams.
  • Medical ICU (MICU)
    During the Medical Intensive Care Unit rotation, fellows are immersed in the management of critically ill patients with life-threatening medical conditions spanning multiple organ systems. Typical cases include septic shock, acute respiratory distress syndrome (ARDS), severe pneumonia, diabetic ketoacidosis, gastrointestinal bleeding, acute renal failure, and multi-organ dysfunction syndrome. Fellows develop expertise in ventilator management, hemodynamic monitoring, renal replacement therapy, and the care of patients requiring complex antimicrobial and vasopressor support. The experience fosters a deep understanding of evidence-based critical care, end-of-life decision-making, and coordination of multidisciplinary care across internal medicine, infectious disease, and pulmonary teams.

Advanced Skill Development: Our programs emphasize the acquisition of advanced clinical skills, ensuring fellows are proficient in the nuances of Critical Care medicine. This includes intensive training in:

  • Ventilator Management: Gaining expertise in managing mechanical ventilators to optimize respiratory support for patients.
  • Hemodynamic and Intracranial Monitor Management: Developing competence in maintaining stable blood pressure and intracranial pressure, which are pivotal for the well-being of neurologically compromised patients.
  • Bedside Procedures: Gaining hands-on experience in essential bedside procedures, including arterial lines, central lines, bronchoscopy, and bedside tracheostomy.

Comprehensive Educational Experience:

Our fellowship programs offer a comprehensive educational journey that extends beyond clinical rotations. Fellows will actively participate in a variety of educational activities, including:

  • Didactic Lectures: Engaging in weekly lectures, guided by experts in the field.
  • Simulation Sessions: Practicing clinical decision-making and teamwork skills in a controlled, simulated environment to enhance real-world performance.
  • Journal Clubs: Regularly convening to critically evaluate and discuss the latest research and advancements.
  • Quality Assurance Discussions: Exploring best practices and quality improvement strategies to enhance patient care.
  • Grand Rounds in Critical Care Medicine: Attending influential sessions led by leaders in the field to further broaden your knowledge base.

  • Applicants must be eligible for the New York State Limited Permit.
  • Preference will be given to those with prior medicine or critical care experience.
  • For those not requiring a visa:
    • U.S. citizens, permanent residents (Green Card holders), and individuals who do not require a visa are exempt from the Step 3 requirement.
    • United States Medical Licensing Examination (USMLE) Step 1, 2 required.
    • Educational Commission for Foreign Medical Graduates (ECFMG) certification required.
  • For those requiring a visa:
    • We are only sponsoring H-1B visas for those already in the U.S. on an H1-B or other non-visitor visa. This is the only visa we sponsor.
    • USMLE Step 1, 2 and 3 required.
    • ECFMG certification required.

While research is not mandatory, it is strongly encouraged, and fellows have the option to participate in ongoing research projects within the divisions of neurocritical care, neurology, and neurosurgery. This active involvement in research serves as an opportunity for those interested in contributing to the advancement of medical knowledge.
