Selecting Documents for Affiliate Faculty
For faculty at Elmhurst Hospital, Queens Hospital Center, and Bronx Veterans Administration, please refer to our checklist and guide in the Summary of Required Documents for requirements on affiliate faculty paperwork submissions.
For faculty at Atlantic Health System, CareMount Medical, Englewood Hospital, Good Samaritan Hospital Medical Center, Jersey City Medical Center, Jewish Home Life Care, and St. Joseph's Hospital, please refer to our affiliate faculty track instructions and forms.
All faculty members are responsible for submitting an updated CV in the required format to their department administrator. Faculty to be nominated to the rank of associate professor or professor must also submit additional information as requested by their department administrator.
Department/division administrators are responsible for assembling completed packets for submission to the Appointments, Promotions and Tenure Office.
